| To help free residents of financial concerns,
the Hospital of Saint Raphael offers a generous and highly competitive salary
and benefits package that includes health coverage, malpractice insurance, an
educational allowance, and four weeks of paid vacation. We will also provide
you with white lab coats and scrubs and take care of necessary laundering. The
benefit plans below represent programs and policies in place as of the start of
the 2006 academic year.

| Malpractice Insurance: |
| During your employment at the
Hospital of Saint Raphael, we will pay for, and provide, all professional
liability insurance. Details are included in the GME contract, which each
house staff member signs before beginning work here. |

| Health benefits: |
The Hospital of Saint Raphael Flexible Benefit program allows residents to choose the benefits that best meet their needs. Elections can be changed once each year during the annual open enrollment period. If you have a qualified life status change (such as marriage or birth) during the plan year, changes must be made within thirty (30) days of the qualifying event.
The Hospital of Saint Raphael gives you Flex Dollars that you may use to help pay for the cost of your benefit package. If your elections cost less than your Flex Dollars, you will receive any excess Flex Dollars as taxable income in each paycheck. If your elections cost more than your Flex Dollars allowance, you pay the difference out of your pay.
You will become eligible for the following benefits immediately upon your employment, provided you have completed the appropriate applications:

Medical insurance:
You may elect medical insurance coverage for yourself, and eligible dependents, from one of the following three plans – Anthem BCBS, Anthem BCBS HMO, and Anthem BCBS POE. Please refer to enrollment materials received during orientation. Contact the Benefits Department at 203 789-3521 with any questions.

Dental insurance:
You and your eligible dependents may be
covered by one of three dental programs offered through Delta Dental:

Option 2, which pays 100% for preventive care, 80%for basic and 50% of major services There is also an orthodontic rider payable at 50% with a $1,000 lifetime maximum per child. PLEASE NOTE ORTHODONTIC IS FOR DEPENDENT CHILDREN UNDER THE AGE OF 19. There is a $50 deductible and $150 for families.

Life insurance:
All benefit-eligible employees are provided with Basic life insurance at 1-times salary (to a maximum of $200,000) at no cost. In addition, benefit-eligilbe employees are offered te opportunity to purchase Supplemental life insurance at 1-times to 5-times salary to a maximum of $500,000. Guaranteed issue amount = $300,000.

Dependent Life insurance:
Benefit-eligible employees may purchase dependent life insurance as follows: Spouse offered $5,000 increments, to lesser of 50% of employee supplemental life amount of $250,000. Guaranteed issue is $30,000. Dependent child (age 2 weeks to 6 months)-$1,000. Deendent child (age 6 months to 19 years, 23 if full-time student)-$10,000.

Long-term disability insurance:
Participation becomes effective the date you complete the appropriate application form. This coverage will be paid for you while you are employed by the Hospital of Saint Raphael. Upon leaving the Hospital of Saint Raphael, you’ll have the option of continuing coverage on a contributory basis. Benefits begin after you have been disabled for 180 days and equal to 80% of income to a maximum of $3,000 a month (i.e., $47,000 income=$2,350 per month. This amount integrates with Social Security, Workers Compensation or other disability benefits.

Short-term disability:
Should you become disabled by illness and are unable to perform your job responsibilities, the Hospital of Saint Raphael will continue your salary for up to six months. However, a statement of disability form must first be received from your physician. This protection begins immediately upon employment.

Retirement program:
The Hospital of Saint Raphael retirement
program consists of:
1. Defined Contribution Pension plan
2. 401 (k) Savings Plan with employer matching contribution
You will be enrolled in the Defined Contribution Pension Plan on the Jan. 1 closest to the completion of one year of service with 1,000 hours. You will receive vesting credit for each year you complete at least 1,000 hours of service and will be 100 percent vested after completing five (5) years of credited service.
You are eligible to participate in the Saint Raphael 401 (k) Savings Plan immediately upon hire, and yyouare immediately vested in your own contributions. Saint Raphael’s will begin to match your contribution, on the first month following the completion of (1) one year of service. dollar for dollar, up to a maximum of 4 percent of your base annual salary. Saint Raphael’s will match your contribution dollar for dollar, up to a maximum of 3% of your base annual salary. You will be 100% vested in the Saint Raphael employer contribution after completing three(3) years of service.

| Other Programs |
Tuition reimbursement:
Following (6) six months of service, benefit-eligible employees are eligible to participate in the tuition reimbursement program. Please refer to the policy or contact the Benefits department for additional information at x 3521.

Employee Assistance Program:
Confidential counseling, assistance and referral services are available to employees (and their dependents) experiencing personal problems, such as alcohol or drug abuse, emotional difficulties, legal concerns, domestic violence and more.
|
Employee Neighborhood Home Ownership Program:
Provides $5,000 of financial help to auqlified employees who want to buy, and live-in a one- to four- family home within the Saint Raphael neighborhood (North Frontage Road to Elm Street and Dwight Street to Norton Street). Before you are eligible, you must work for Saint Raphael’s for at least two years and meet several other guidelines.
